closed"?column?" appears as the header for last foil in downloaded data

Description
when downloading an assessment with a mix of questions, we sometimes see the ?column? label show as the last foil.
This will happen when the "first" question that we select does not have a foil (say there are A-D options for this first question, but other questions include a 5th (E) option).
This should not affect the returned data, except that it may be confusing. Because of how the download query is generated, it's not immediately clear how we can prevent this from happening (especially since every assessment or set of assessments can be very different).

i've addressed this for the metadata fields - which is crucial for generating the "transposed" data download

resolved as best i can at this point. at least it is not preventing us from accessing some of the crucial transpose functions now
