Undo doesn't work for image manipulation + other contexts

Description
Dragging an image onto a question doesn't seem to work in terms of undo. The buttons become active, but they don't actually do anything.
UNDO fails to reverse the following changes:
- Image added by user
- Image disposal does not revert "show images" button to "hide images"
- Add question
- Choose question from source
- Changing a question type
UNDO works in the following cases:
- copy/paste/cut
- edit mode to done mode
- dispose question and then add question
- adding/deleting a choice is reversible
- other edit features such as bold, italic, superscript, and subscript
