Description
The second screen of the profile wizard does not have a default focus of the
cursor, causing the user to have to click on the username box instead of just
hitting the tab key. ideally, the username text box should have the default
focus so that the user does not have to do anything to start typing there.

set default focus to the username; also added "unaffiliated" to the list of
organizations.
