Not clear what this bug refers to. My best guess is that it is a combination
of business rules as in bug 716 with creation of forms for this task as in
some other bug assigned to ML. Accordingly, I shift this bug back to Mike to
create the forms. I will do quality control on the forms.

only one bug as a bucket for this change. First Lee design, then Peet evaluate,
then Farrell implement, then Lee check that it works.

changed from components that are to be deleted to "misc" so that bugs don't get
