split BASIC INFORMATION section into BASIC INFORMATION and SUBMITTER NAME

Replaced <tr class="sectbody"> with <tr> this removes the bacground color that is
a problem with non-bolded text.
Only the rows have the <tr class="sectheader"> will have background color

The guide on how to complete dr entries was updated to describe the current entryform.tmpl, and to fix bug 1302.

Revised the registry guide to include descriptions of the new fields, and
made some minor formatting changes.

This is the merge from the branch WEB_UI_REDESIGN_BRANCH back into the head.
The only files that have been removed are those in lib/style/eml2; other files
that have been cvs removed (only from lib/ and lib/style) have been re-added
elsewhere (i.e. moved from one directory to another)....
